59M Abdominal pain, vomiting, slurred speech, weakness, severe Hyponatremia, Toddy potomania, PaJR

C/o pain abdomen since 10 days
Loss of appetite since 10 days
Vomitings since 8 days 
Unable to walk properly since yesterday 
Slurring of speech since yesterday 
Hiccups since yesterday 
Fever since yesterday 
Patient was apparently normal 10 days back then he developed abdominal pain, insidious in onset, non radiating, at peri umbilical region for which he consumed Eno, and the pain didn't subside, so he went to a hospital where he found out he had renal calculi, and they advised conservative management and gave medication, ketorolac, cefuroxime
Following which he developed vomiting 3-5 episodes/d, following food consumption, watery consistency, non-bilious non-blood stained.
Then as the symptoms doesn't resolve he went to another hospital where they found at the calculi was at pelvic ureteric junction, and he was given nsaids again, this time mefinimic acid, dicyclamine, tamsulosin also 
And the pain still didn't subside so Tuesday night they gave him toddy approximately 500 ml
Then on Wednesday morning he was unable to walk, with generalised weakness, slurring of speech, hiccups 
No h/o involuntary movements, deviation of mouth, tongue bite, rolling of eyes 
N/k/c/o T2DM, HTN, CVA, CAD, Epilepsy, TB, Thyroid disorder 
Chronic alcoholic since 40 years 180ml/d
Tobacco chewing.
