THIS PATIENT DATA WAS GATHERED BY ROLL NO 9, BATCH 2023 AS PART OF HER COMPETENCE BASED LEARNING PORTFOLIO EVALUATION PREPARATION

THIS IS THE CASE OF 56F, HOMEMAKER WITH HEADACHE SINCE 2 MONTHS AND BLURRING OF VISION IN THE LEFT EYE SINCE 2 YRS

56 years / Female / Homemaker

Here is a set of Socratic questions based on the case report to guide medical students through clinical reasoning, history-taking, and differential diagnosis:
1. History of Present Illness (HPI) & Symptom Chronology
 * Question: The patient presents with a headache for 2 months, but a history of left eye blurring for 2 years. How do you clinically approach a patient presenting with symptoms of vastly different chronicity (acute/subacute vs. chronic)? Are these two complaints likely related, or should they be investigated as independent processes?
 * Question: When evaluating the patient's headache of 2 months duration, what key "red flag" features would you specifically screen for to rule out secondary causes of headache (e.g., intracranial mass, elevated intracranial pressure, or temporal arteritis in this age group)?
2. Past Medical History & Pharmacology
 * Question: The patient has a known history of hypertension managed with Cilnidipine (10 mg). What is the unique pharmacological mechanism of cilnidipine compared to traditional L-type calcium channel blockers, and could her chronic headache or visual symptoms be related to her blood pressure control or medication side-effect profile?
 * Question: Given her history of right eye cataract surgery 4 years ago and current left eye blurring, what physical exam maneuvers and ophthalmologic assessments should be prioritized to differentiate between refractive error, cataract progression, retinopathy, or a neuro-ophthalmic pathology?
3. Surgical & Past History Integration
 * Question: The patient notes a history of hysterectomy 30 years ago. While distant, why is it still clinically relevant to document past surgical history accurately in an older adult, and how does it fit into the broader picture of her baseline health status?
4. Diagnostic Reasoning & Provisional Diagnosis
 * Question: The provisional diagnosis mentions a "Hypertensive with blurring of vision". How does chronic hypertension specifically affect the microvasculature of the eye, and what specific fundoscopic findings would you expect to see if hypertensive retinopathy is contributing to her visual changes?
 * Question: Based on her complete presentation (56-year-old female, chronic headache, asymmetric visual blurring, and hypertension), what would be your structured differential diagnosis, and what initial diagnostic investigations (laboratory or neuroimaging) would you order next to confirm or refute your hypotheses?
