THIS PATIENT DATA WAS GATHERED BY ROLL NO 17, BATCH 2023 AS PART OF HER COMPETENCE BASED LEARNING EVALUATION PORTFOLIO PREPARATION
THIS IS THE CASE OF 61F WITH KNEE PAIN SINCE 5 MONTHS AND ANTALGIC GAIT
*Case Details:*
*Age/Sex:* 61/F
*C/C:* Pain in left knee since 5 months
*HOPI:*
Patient was apparently asymptomatic 5 months ago. 5 months ago she fell in the gutter due to impact of strong winds and fractured her knee.
Since the incident she had pain. It was insidious in onset, progressive in nature. It is aggravated on walking, relieved on joint relaxation. She took herbal medicine and also physiotherapy which was of no use. Pain is dull. She's experiencing pain on flexion of knee. (Antalgic gait) 👇
*Past H/O:* N/K/C/O HTN, DM, Asthma, epilepsy.
*Personal H/O:*
- Attained menopause 5 years back
- Diet - Mixed
- Appetite - Normal
- B&B - Regular
- Sleep - Sound
*Family H/O:* No significance
*General Examination:* Pt is conscious, cooperative, well oriented to time/place/person
Gen Exam:
PICCLE - Absent
Temp - 98.7°F - Afebrile
BP - 120/80 mmHg
PR - 82 bpm
Sys Exam:
CVS - S1S2+
CNS - NVBS
Provisional Diagnosis: Knee OA
Here is a set of Socratic learning questions tailored for medical students to encourage clinical reasoning and structured analysis based on this case report:
Clinical Reasoning & Differential Diagnosis
* Pathomechanics: How does an acute traumatic event (such as a fall triggered by strong winds) accelerate or unmask underlying degenerative changes in a 61-year-old female, and how do we clinically differentiate acute post-traumatic injury from an acute exacerbation of primary osteoarthritis?
* Gait Analysis: The patient demonstrates an antalgic gait. What specific phases of the gait cycle are typically shortened in patients with knee osteoarthritis, and what compensatory biomechanical mechanisms are at play?
History & Risk Factor Assessment
* Endocrine & Musculoskeletal Health: The patient attained menopause 5 years ago. How does post-menopausal estrogen deficiency impact bone mineral density and articular cartilage metabolism in the context of degenerative joint disease?
* Interpreting Chronicity: The pain has been progressive over 5 months with insidious onset following trauma. Why is it important to screen for mechanical symptoms (such as locking, catching, or giving way) when evaluating chronic post-traumatic knee pain?
Management & Diagnostic Approach
* Diagnostic Imaging: Given the provisional diagnosis of knee osteoarthritis, what are the standard radiographic views required to assess joint space narrowing, subchondral sclerosis, and osteophyte formation?
* Therapeutic Interventions: The patient tried herbal medicine and physiotherapy without relief. What is the multimodal, evidence-based stepwise approach for managing knee osteoarthritis in older adults before considering surgical consultation?
