45M Right hemiparesis with sensory loss of pain and touch gradual onset 1 year
On examination:
peripheral polyneuropathy
Etiology: symptoms actually noticed ten years back after a febrile episode and were very gradually progressive with maximum progression in his right half of body.

Many other overlapping features
Severe hip pain gradually progressive since 6 months
Shortness of breath since 6 months

Previous complaint of pain in the limbs has subsided.
