Here is a set of Socratic questions tailored for medical students based on the clinical case of the 45-year-old female with back pain:
1. History of Present Illness (HPI) & Pain Characteristics
 * Question: The patient reports a 2-year history of insidious, progressive lower back pain that aggravates with physical work. How does this chronic, activity-related onset help you differentiate mechanical back pain from inflammatory or neoplastic causes?
 * Question: The patient had a fall 2 days prior to presentation. Given her chronic background pain, how do you determine whether this recent fall is an acute precipitating injury (or fragility fracture) or merely an incidental finding?
2. Risk Factors & Social History
 * Question: The history notes a 30-year history of toddy consumption (2 glasses/day). What potential impacts can chronic alcohol use have on bone mineral density, muscle recovery, or nutritional status (such as calcium and Vitamin D absorption) in a middle-aged woman?
 * Question: As a 45-year-old female presenting with chronic musculoskeletal pain, what endocrine and metabolic milestones (such as menopausal status) should you actively inquire about, and how might they influence bone and joint health?
3. Clinical Examination & Triage
 * Question: Her vitals are stable (BP: 120/80 mmHg, PR: 70 bpm, Afebrile, SpO2: 98%) and her general physical/systemic exams are unremarkable. What "red flag" symptoms are notably absent here that would otherwise urgently prompt neurosurgical or advanced oncological imaging?
4. Differential Diagnosis & Reasoning
 * Question: The provisional diagnosis is listed as Osteoarthritis. Considering her age, gender, chronic mechanical symptoms, and the weight-bearing nature of the spine, what specific features support this diagnosis, and what other differential diagnoses (e.g., spondylolisthesis, lumbar canal stenosis, or early spondyloarthropathy) should remain on your radar?
