PARTICULARS:

Age: 50 years

Occupation: Teacher

Address: Coochbehar, West Bengal

CHIEF COMPLAINTS:

1. Frequent acidity – Triggered by fasting and spicy food

Past medical history: Duodenal ulcer (2011 and 2002)

2. Constipation

Recent history: Diagnosed with thrombosed piles a few months ago; resolved with medication

3. Knee pain – Radiates toward the hip and legs

History of arthritis diagnosed a few years back

 

Obstetric History:

Parity: 1

C-section in 2001

Currently in the perimenopausal phase

(Past history of PCOS, now resolved) 

Current Medications:

1. Olmezest 20 (antihypertensive)

2. Calcium tablets

3. Vitamin D supplements

24-HOUR ROUTINE

6:00 AM: Wakes up

8:00 AM: Tea and biscuits

9:30 AM: Breakfast (rice)

10:00 AM: Leaves for school

1:30 PM: Tiffin (muri or rice)

7:00 PM: Tea and snacks

10:00 PM: Milk, oats, banana

11:00 PM: Sleep
