Here are Socratic questions based on the clinical case report to guide medical student learning, categorized by clinical reasoning domains:
1. History Taking & Risk Stratification
 * Question: The patient is a 60-year-old daily wage worker who reports a 1-year history of bilateral knee pain. How does his occupation as a manual laborer influence your differential diagnosis for his joint pain?
 * Question: He has a 20-year history of alcohol consumption and smoking (along with past toddy consumption). Why is it clinically important to detail the exact duration, frequency, and units of past substance use in a patient presenting with musculoskeletal complaints?
2. Pathophysiology & Clinical Correlation
 * Question: The patient has a known history of Diabetes Mellitus and Hypertension for 1 year, alongside bilateral knee pains and right eye pain. Could there be a systemic or microvascular link connecting his chronic hyperglycemia/hypertension to his joint and ocular symptoms?
 * Question: The provisional diagnosis is Osteoarthritis. What are the hallmark mechanical characteristics of osteoarthritis pain, and how would you distinguish it clinically from inflammatory arthropathies (like rheumatoid arthritis)?
 * Question: The patient also reported right eye pain. In a patient with joint pains and systemic comorbidities, what potential ophthalmologic complications or systemic inflammatory conditions must you rule out?
3. Pharmacology & Management
 * Question: He is currently prescribed Telmisartan, Amlodipine, and Coversyl (perindopril) for hypertension. What class of medications does Coversyl belong to, and what are its common side effects that could overlap with generalized body aches or fatigue?
 * Question: When considering pain management for a 60-year-old male with hypertension, diabetes, and suspected osteoarthritis, what are the major pharmacological and renal safety precautions you must consider before prescribing Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)?
