45M Right hemiparesis with sensory loss of pain and touch gradual onset 1 year

On examination:

peripheral polyneuropathy

Etiology: symptoms actually noticed ten years back after a febrile episode and were very gradually progressive with maximum progression in his right half of body.

PPM 1 - Alright just supports our neurological localization while etiology remains the virus that caused his fever and weakness 10 years back.

PPM 1 - Reviewed him now again in the OPD:

Many other overlapping features

Severe hip pain gradually progressive since 6 months

Shortness of breath since 6 months

Fresh problem: Pruritus with previous Pedal edema

Previous complaint of pain in the limbs has subsided.
