HERE WE DISCUSS OUR PATIENT'S PROBLEMS THROUGH SERIES OF INPUTS FROM AVAILABLE GLOBAL ONLINE COMMUNITY EXPERTS WITH AN AIM TO SOLVE THOSE PATIENT'S CLINICAL PROBLEMS WITH COLLECTIVE CURRENT BEST EVIDENCE BASED INPUTS.
**CASE REPORT: 48F DIABETES MELLITUS WITH RECURRENT FOOT COMPLICATIONS**
**INTRODUCTION**
The management of diabetic foot ulcers (DFU) in patients with long-standing Type 2 Diabetes Mellitus (T2DM) remains a significant clinical challenge, particularly when complicated by Chronic Kidney Disease (CKD) and poor glycemic control. This report follows a 48-year-old female from Telangana with an 8-year history of T2DM, who progressed from a simple blister to septic shock and amputation, and now presents with a recurrent ulcer.
**METHODS**
This case was documented through longitudinal clinical logs (2024–2026) and external hospital records. Data includes serial blood glucose monitoring, renal function tests (Serum Creatinine), and clinical photography analyzed via AI-assisted wound morphology assessment.
**RESULTS**
* **Initial Presentation (April 2024):** Patient presented with a left great toe wound, fever, and hypotension (70/50 mmHg). Investigations revealed Hb 9.4 g/dL, TLC 23,900/mm³, and Serum Creatinine 3.1 mg/dL.
* **Intervention:** Disarticulation of the left great toe was performed on April 13, 2024. Post-operative course was complicated by septic shock requiring dual inotropes (Noradrenaline/Vasopressin).
* **Long-term Follow-up (2025–2026):** Glycemic control remained volatile (RBS ranging 68–415 mg/dl). Serum Creatinine stabilized at 2.0 mg/dL (May 2025).
* **Current Status (March 2026):** AI analysis of recent images confirms a new 1.5 cm ulcer on the medial aspect of the left hallux (amputation site) with yellow-brown slough and periwound hyperkeratosis, suggesting worsening diabetic neuropathy and peripheral arterial disease.
**DISCUSSION**
The recurrence of the ulcer post-amputation highlights the "mole-hill to mountain" phenomenon in diabetic care. Despite surgical intervention, the underlying drivers—uncontrolled hyperglycemia and CKD—persist. The patient’s advocate reported skipping pre-lunch insulin doses, leading to significant post-prandial spikes (e.g., 415 mg/dl), which directly impairs wound healing.
**SOCRATIC QUESTIONS FOR CLINICAL REFLECTION**
1. **Pathophysiology:** Given the patient's Serum Creatinine of 2.0 and persistent proteinuria (+++), how does her renal status influence the choice and dosing of her insulin regimen?
2. **Behavioral Barriers:** The patient advocate frequently skipped pre-lunch insulin because "pre-meal sugars were normal." How can we effectively shift the caregiver's mental model from "reactive" (treating current high sugar) to "proactive" (preventing post-meal spikes)?
3. **Wound Evolution:** AI analysis noted macerated tissue and slough but no frank necrosis. What clinical signs would differentiate this chronic non-healing state from a new acute infectious process requiring urgent surgical debridement?
4. **Systemic Integration:** With a history of septic shock and multiple organ dysfunction, what is the patient's current cardiovascular risk profile, and how does the Grade 1 diastolic dysfunction noted in her 2D-Echo impact her long-term prognosis?

💡 **CLINICAL NOTE**
The patient has developed a new ulcer at the previous amputation site. This recurrence, coupled with a Serum Creatinine of 2.0 and inconsistent insulin adherence, places her at high risk for further limb loss. Urgent reinforcement of the "Basal-Bolus" rationale is required for the caregiver.
48F DM Uncontrolled Sugars Left Great Toe Amputation
A 48 year old female resident of (Narketpally Mandal) who is a homemaker came to casualty with complaints of Wound over left foot since 1 week.
Patient was apparently asymptomatic 8 yrs back, then developed generalized weakness for which she went to hospital and diagnosed to have DM for which she started using medication Tab. METFORMIN 500MG PO/OD & Tab. GLIMEPERIDE 1MG PO/OD. She was using tablets regularly. In between she visited our hospital for 2-3 times then her FBS was <250.
FEBRUARY 2024
She developed a fluid filled bleb over her left Great Toe which ruptured spontaneously and healed after dressing done at local hospital. No H/O of fever at that time.
10th APRIL 2024
She developed a wound near left great toe associated with swelling of left LL and fever for which she came to OPD. Advised for admission but they denied admission.

AS PATIENT CAME WITH FOLEYS IN SITU, FOLEYS WAS CHANGED ON THE DAY 1 OF ADMISSION AND DECATHETERISED ON DAY 5 OF ADMISSION.
ON ARRIVAL PATIENT BP-90/50 MMHG WITH NORAD 4 ML/HR GRBS- 165 MG/DL ON INJ. INSULIN HAI SC/TID.

If we give the soluble insulin much earlier, we risk severe pre-meal hypoglycemia if the absorption is slightly different that day or if the meal is delayed for any reason.
This is precisely why we've decided to switch her to a basal-bolus regimen with Lantus. It provides a stable, predictable basal foundation, which is far safer than trying to guess the timing of an unreliable regular insulin in a patient with significant CKD. Once we stabilize her fasting glucose with the Lantus, we can then address post-meal spikes with a small, appropriately-timed dose of rapid-acting insulin.
