THIS IS AN ONLINE E LOG BOOK TO DISCUSS OUR PATIENT'S DE-IDENTIFIED HEALTH DATA SHARED AFTER TAKING HER SIGNED INFORMED CONSENT. HERE WE DISCUSS OUR PATIENT'S PROBLEMS THROUGH SERIES OF INPUTS FROM AVAILABLE GLOBAL ONLINE COMMUNITY EXPERTS WITH AN AIM TO SOLVE THOSE PATIENT'S CLINICAL PROBLEMS WITH COLLECTIVE CURRENT BEST EVIDENCE BASED INPUTS.
The patient is a 55 year old female, stayed in our hospital campus for over a year from 2019 and later in 2020 when she donated her kidney for her son who was transplanted here.
[29-03-2025 07:25] PPM 3: CHEIF COMPLAINT -
Chest pain since past 5 days
HISTORY OF PRESENTING ILLNESS -
The patient was apparently asymptomatic 5 days back and then suddenly she developed chest pain which was sudden in onset, continuous, burning types of pain, which was located in the mid sternal region radiating towards the right side below the breast. The chest pain was associated with SOB (NYHA Class II) which worsened on chest pain and nausea (4 episodes) which usually happens after a meal. Chest pain temporarily relieved on unknown medication.
PAST HISPORY -
N/K/C/O HTN, DM, Thyroid disorders, TB, asthma, epilepsy, CAD, CVA.
Surgical history -
Patient currently has her right kidney, the left kidney was donated to her son on 16th April 2021
FAMILY HISTORY -
Husband was a K/C/O HTN, passed away in 2020 due to kidney failure
Husband's father, sister and first cousins were a K/C/O HTN, and passed away due to kidney failure
Son had a kidney transplant surgery in 2021 and die in October 2022 due to kidney failure.

Going by your history I'm still considering a cardiac cause for her chest pain and once we can rule that
out on our follow up of her daily hourly activities which you can ask her advocate to share here hourly,
we may conclude that it's a non cardiac esophago-gastric dysmotility pain.

I talked to her sir, the history that I took was a day prior to her discharge ie. 28th. She was discharged on
29th. Her symptoms were relieved on medication. She has a few tests left to be done, she'll get them
done soon.
About the informed consent sir, I reached out to her multiple times but as she was in the hospital she
couldn't get it done. I talked to her right now and she told that she'll get it done by tomorrow.
